account funds are to be deposited in.
Fed Tax ID, If sole proprietor enter
Social Security Number
Click Check Box for Form 1099, note checkmark
may not appear, proceed.
Additional Information field, enter desription of
products sold or service provided.
Page 3 - Review Terms and Conditions, Click on I AGREE.
Page 4 - Answer all Identity Verification questions,
click SUBMIT to send application for approval.
